Important Info:

● Print the Medical History and Physical (front and back). Bring it with you to the physical night.

● You will need to have the UIL Medical History form filled out and signed by your parent/guardian before arriving on the day of your assigned physical night.

● VAISD requires an ANNUAL physical in order to participate in athletics, band, or cheer.

● The link to access the forms needed to participate in VAISD athletics is located here.

○ Please follow the instructions on the main page and be sure to upload a copy of your current physical.

● You will be considered ineligible until you have completed the Medical History and Van Alstyne/UIL Signature page through Rank One. A copy of your physical must also be uploaded and approved through Rank One.

○ All of these forms must be submitted before you can participate in any tryout, practice, scrimmage or contest for VAISD/UIL.
